כותרת קצרה
שמות העוררים
שמות המשיבים
נושא הערר
כתובת הנכס (אם רלוונטי)
תאריך דיון (אם ידוע)

אחרי יצירת התיק, הצע למשתמש להעלות מסמכים עם /upload-doc.
